How Can We Help?

How much will my copayment be?

Your copayment will depend on your insurance. We check your insurance eligibility for each visit and will let you know what your copayment is at the time of your visit. You may also call us prior to your appointment if you wish to know this information before hand. 

What is your new patient procedure?

You will be required to fill out registration forms at check-in, or you may fill these out via our patient portal. Please bring your insurance card, ID, and immunization record. 

What if I'm running late or need to cancel?

Please notify us of if you are running late or need to cancel. There is a 15 minute grace period, but we will try to accommodate you as best as we can. However, if the schedule is really busy, we may need to reschedule your visit. 

I don’t have my insurance card with me, can I still receive treatment?

As long as you can provide us with the information to verify  your insurance eligibility, you can receive treatment. 

What form of payment do you take?

We take cash and credit card.

If I don't have insurance can I still receive treatment?

We can still serve you if you currently do not have insurance. Please call us for pricing inquiries.
